Please check the list of required documents the school needs to be able to register your student...
- Original birth certificate
- A copy of the students most recent physical and immunizations
- If student is not living with parent, proof of legal guardianship must be provided
- DCF 603 form if a foster placement
Proof of Thompson Residency (provide one):
- Mortgage statement or rental/lease agreement
- Utility bill (such as gas, electric, phone)
- A Certificate of Residency form needs to be filled out and must be notarized if the student and the parent/legal guardian will be residing with another Thompson resident.
